How to request an appointment:
For a new or ongoing medical issue
Same day / urgent appointments
For urgent medical issues which require a same day appointment, please contact us during our opening hours. If you require a same day appointment, we will ask you a series of questions. Our Triage team, led by GPs, will use your responses to determine which is the most appropriate health professional or service to help you.
We will contact you to offer you an appointment, provide advice or direct you to the most appropriate NHS service.
Routine GP appointments
These are released weekly and availability is limited by our overall capacity. If our GP appointment book is full, visit 111.nhs.uk (or for children visit the Healthier Together what0-18.nhs.uk website) for expert medical advice.
Your local pharmacy may be able to help you without you needing to see us first. Find out about Pharmacy First here.
Other types of routine appointments
If our GP appointment book is full, you can still contact us to book:
- a routine nursing appointment (e.g. for wound care, vaccinations, health reviews and smear tests)
- a blood test a healthcare professional has asked you to arrange.

Outside of opening hours call 111 or in an emergency dial 999.
You will be asked the same questions whichever way you contact us.
The questions we will ask you have been written by GPs to gather the information needed to assess you. Your answers will be sent directly to our Triage team, which is led by GPs. They will use your responses to determine the most appropriate health professional or service available to help you.
Availability of Anima
If our GP appointment book is full, you will be unable to submit an Anima medical request.
We aim to accept online medical requests again from 8:00am the next working day.
You can still contact us by phone or visit the practice during opening hours.
Alternatively, visit 111.nhs.uk.
Anima admin requests can be submitted Monday – Friday, 8am – 8pm.
Use an Anima admin request to ask for sick notes, medical forms or health records, etc.
How to use Anima
You will need to sign up or login to your Anima account to start your request.
You will then be able to track the status of your requests using your account.
Using Anima for a child or dependant
You need to add your children and any adult dependants to your Anima account.
Make sure you have selected their name before you submit the request.
What will happen next?
We aim to respond to your medical request by the end of the next working day.
We may offer you:
- a same-day appointment
with the most appropriate healthcare professional either by telephone or in person at the surgery.
This could be with a GP, nurse or another health professional.
If you think your problem needs a same-day appointment, please be prepared to be available. - a ‘self-book’ appointment for another day
with the most appropriate healthcare professional either by telephone or in person at the surgery.
This could be with a GP, nurse or another health professional.
We will send you either a text message with a ‘self-book’ link or call you to arrange an appointment. - a new prescription or a change in your medication
- a test for further investigation, e.g. a blood test
- a referral to another specialist
- details of other NHS services who can assist you, e.g. your Community Pharmacy if suitable
- advice about self-care actions you can take to aid your recovery.
Consultations by phone or by text can be more flexible and often mean our healthcare professionals can help you sooner.
We may ask you for more information
We will contact you if we need further information to help us to assess you. We will either send you a message via email and/or text. You will be able to view messages and directly respond in your Anima account. We will call you if you cannot receive emails or texts.

Blood test appointment
You must be invited to book a blood test to make an appointment.
We can only provide blood tests for patients aged 16 years and older:
Patients aged 16 years and over
Patients aged 16 years and over can book a blood test appointment if this has been requested by us or the hospital.

If the hospital has requested the test, you must bring the blood test request form with you.
Patients under 16 years
We are unable to provide blood tests for children aged under 16 years.
Patients under the age of 16 need to arrange a blood test appointment at the hospital.
If we have requested the blood test, we will provide you details of how to book a test appointment for a child.
If the hospital has requested the blood test, please contact them directly to arrange the appointment:
Children aged 0-13 years: Appointments must be made over the telephone by calling 07707 296876. Phone lines are open Monday to Friday, between 9am and 4pm.
Children aged 14 year and over: can use the hospital’s adult blood taking service. Further details are available at: Blood Testing (porthosp.nhs.uk)
Nursing team appointment
Appointments with our nursing team can be made for issues such as wound dressings, removal of stitches, travel immunisations and advice and other nursing team clinics.

- You can use your NHS App or Patient Access account if you have been invited to book a cervical screening or B12 injection.
Health review, cervical screening or vaccination appointment
You will be contacted when it is time to book your:
- Cervical screening (smear test)
- Health review for a long-term condition (e.g.: high blood pressure, diabetes, chronic kidney disease, COPD and asthma)
- Medication review
- Cancer care review
- Winter flu & Covid vaccinations for eligible patients
- Shingles and Pneumococcal vaccinations for eligible patients
- Baby & childhood health check and vaccinations

Help with your appointment, home visits and more
If you need help with your appointment
Please tell us:
- if there is a specific doctor, nurse or other health professional you would prefer to respond
- if you would prefer to consult with the doctor or nurse by phone, face-to-face, by text or email
- if you need an interpreter
- if you need a chaperone
- if you have any other access or communication needs.
Home visits
If you are housebound and need an appointment, we will try to provide a home visit. We will phone you first to understand what you need and how urgently you need to be seen.
To request a home visit, it’s helpful if you call 01329 845777 before 10am. Please provide full details to enable us to schedule house calls.
Our doctors and other healthcare professionals will only visit you at home if they feel that your medical condition requires it.
If you need help when we are closed
If you need medical advice, use NHS 111 online or call 111.
NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.
Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.
Visit your local community Pharmacy. Pharmacists are experts in medicines and qualified healthcare professionals who can help with a range of minor illnesses, such as coughs, colds, sore throats, rashes, cystitis, tummy trouble and aches and pains. Find a pharmacy at NHS: Find a pharmacy.
